US tax forms needed for providers regardless of country

Forms W-8BEN and W-9 for individuals

Service providers who receive payment through an IFCSN grant are required to fill out one of the following two forms and send it to IFCSN in order to comply with US tax requirements:

  • W-8BEN is a form to be filled out by citizens of countries other than the US residing outside of the US who do not have a US Social Security Number.
  • W-9 is a form to be filled out by US Citizens and US residents who have a US Social Security Number. (You will have this SSN if you have lived in the US.)

Whichever form is relevant it should be completed in English.

Forms W-8EXP for Organisations

Organisations offering Christian Science nursing care are also required to fill out a form: W-8EXP. When registering as an organisation in the IFCSN secure portal to request payments you will be guided to fill out the appropriate form.

If you’re providing your services in a non-US country, you need to fill out a W-8BEN form to claim an exclusion from being taxed in the US.

A W-8BEN form includes three parts. Most providers normally need to fill out only 2 parts:

  • Part I is identification data about the payee.
  • Part III is the certification, or signature, confirming that the ID info is correct.

Part II, the part where treaty benefits are claimed, is only to be filled out if services are also provided in the US and the individual has no US-Social Security number. IRS Form W-9, also known as a W9, is one of the many tax documents required by the IRS (Internal Revenue Service) to help accurately estimate the taxes owed by contract workers in a given tax year. It’s a request for information about the contractors paid by a company and establishes that they are not an employee but rather a contractor and must pay all their own taxes on this income.

In general, these forms are valid for three years. You will be asked to submit it once every three years to continue to receive payment from The International Fund for Christian Science Nursing.
